Youth Heroism: Recognition of Mark Avetikyann
Mark Avetikyann received the 'Future of Ukraine' medal from the President for his bravery and patriotism.
The Diia portal has reintroduced the "eBaby" service, allowing parents to quickly apply for up to 9 state services with a single application.
The Odesa City Council's Administrative Services Center is also resuming document acceptance for the "eBaby" electronic public service.
Parents may apply for the eBaby service if:
- the child does not yet have a birth certificate;
- the child was born in a medical institution in Ukraine;
- the parents are citizens of Ukraine.
Required documents for the application:
- marriage certificate (if available),
- passports of both parents,
- TIN (Taxpayer Identification Number) of both parents,
- unique record number from the State Demographic Register for at least one parent,
- electronic medical birth certificate or medical birth certificate (form No. 103/o),
- certificate of refusal from the one-time natural assistance "Baby Package" from the maternity hospital,
- bank account number of the child benefit recipient (the parent on parental leave),
- "PrivatBank" card,
- special account for the "Baby Package" (if refusing the natural assistance).
To receive the comprehensive service, parents of newborns must make a preliminary appointment in the electronic queue.
